The following are links to screening questionnaires that you can complete on your cell phone or computer. After completing the questionnaire you will see a PDF that you can print or save for your use. On each report there is a Key Code that you can give to an AMHA psychotherapist or clinical counselor who can upload your results and review with you during your first appointment.
